The information that Barrie Hypnosis Centre collects on its customers include name, address (home, office and cell), gender, age, birthdate, history, business description, marital status, number of children, email, personal information pertaining to their particular situation.
1. Accountability. The privacy officer of Barrie Hypnosis Centre is the owner and sole proprietor, Janet Marvin. The responsibility of the privacy officer is to respond to enquiries, complaints, update information, understanding the legislation.
2. Purpose. Information is gathered from clients for the following purposes:
To provide appropriate hypnotic services;
To generate an invoice;
To be able to contact the individual for upcoming appointments;
To provide follow up services when appropriate and requested.
3. Consent. Consent is obtained from each individual client by giving them a form that they sign at the time of their first appointment.
4. Limit. All information received is pertinent and unnecessary information is not obtained.
5. Limit Use, Disclosure & Retention. Information gathered is retained in hard copies and locked in a file cabinet when not in use in a home based office. Only Janet Marvin has access to information gathered. Information will be destroyed by way of shredding after ten (10) years or upon request of the client.
6. Accuracy. Information is gathered by the client filling in a form and by way of verbal communication with Janet Marvin.
7. Appropriate Safeguards. The hard copies are stored in a locked filing cabinet in the home office of Barrie Hypnosis Centre and are locked when not in use. Only Janet Marvin has access to the information and no unauthorized parties can access information.
